Abstract

Providing end-of-life care for individuals with speciÀc or multiple chronic conditions poses several challenges and complexities. These challenges arise from the unique needs and circumstances of each patient, as well as the interdisciplinary nature of end-of-life care. Here are some of the key challenges associated with providing end-of-life care for individuals with chronic conditions and across specialties:
