The values and skills needed by the students in this era of dramatic changes in healthcare delivery system has emphasized further the role of nursing in different settings.

The aim of writing this Practical Record Book of Community Health Nursing is to help the nursing students to have handful of clinical requirements guide and to facilitate them to complete clinical posting with effective utilization of given learning experiences. It serves as a clinical guide for the students to understand comprehensive nursing care by gathering client information, performing nursing assessment and documenting the provided nursing care.
